This is the form to be used by potential new clubs to apply to the league. This can filled out on screen and then either e-mailed to Pat Ansell or printed and posted. Once submitted, clubs will be contacted to confirm our receipt of the application. If you don't hear from us within a few days, please call us to find out it it has been received.

Player registration forms for 2008/09 season. Free of charge - download as required. Please cut in half before sending in.

Used for taking the names, DOBs and signatures of opposition players in the case of a query on a matchday.

This is the form for referees to register with the league, providing their contact details and their available dates for the season.

Used for referees to report the match details to the referee's secretary. Note: this is version 1 of this document. Some users have had problems with v2 hence the re-introduction of v1. An e-form similar to the e-card for match results is forthcoming.

This form is used to transfer a player from once club to another. Make sure that all parts (excl. part 4) are complete before sending to Registrations Secretary.
